Terms & Conditions – Elevate Myotherapy
Appointments
Appointments can be booked online and are subject to availability. If you are unsure which appointment type to select, an initial consultation is recommended.
Cancellations & Rescheduling
If you need to cancel or reschedule an appointment, please provide notice via email or SMS.
A minimum of 24 hours’ notice is required for cancellations or rescheduling.
Cancellations or changes made with less than 24 hours’ notice will incur a 50% fee.
Cancellations made with less than 2 hours’ notice, or failure to attend a scheduled appointment, will incur a 100% fee.
These policies are in place to allow appointment times to be offered to other clients and to respect practitioner time.
Payments
Payment is required in full at the time of the appointment unless otherwise agreed. An invoice will be provided for claiming purposes.
Health fund rebates are available; however, on-the-spot claiming is not currently available at this time. HICAPS is coming soon.
Health Information
Clients are responsible for providing accurate and complete health information to ensure safe and appropriate treatment. Failure to disclose relevant information may impact treatment outcomes.
Scope of Practice
Myotherapy services are not a substitute for medical diagnosis or emergency care. Where appropriate, you may be referred to another healthcare professional.
Changes to Terms
Elevate Myotherapy reserves the right to update these terms and conditions at any time. Any changes will be reflected on this website.
